These are the options you have in supplying information.

1) Tell me about one, two or three of you health complaints. Tell me how long you’ve had each of them and what you’ve done for them so far.

2) Or fill out the new Patient Paperwork and either fax (619-265-1469) or email (docmmm@cox.net) it to me.

3) Giving me more information about your health situation would help me even more in determining how much I can or can’t help. Fill out #2 and the Health Assessment Questionnaire and get it to me with email or fax.
